Case Management Services

Upon admission, all patients are screened by a registered nurse case manager, who works closely with you, your physician, your family, your healthcare team and your health insurance carrier to ensure that the care you receive at St. Mary's Hospital best suits your needs. Under the direction of your physician, your case manager will develop and arrange a safe and appropriate plan to continue your recovery at home after discharge. Clinical insurance decisions that negatively affect you or the hospital are also appealed by a member of the Case Management Department.
